(1) Hepatitis B – 1
st dose should be administered
at birth – series can’t be completed prior to age 6 mo. or 24 wks.
(2) DTaP (Diphtheria, Tetanus, & acellular Pertussis),
primary series X 4 & booster dose @ age 4 yrs – not for persons 7 yrs. &
older
(3) Td (age 7 & up)
(4) Tdap (Boostrix / Adacel) ages 10 or 11 and up
(5) DT (for persons who are unable to take vaccines with
Pertussis component)
(6) HIB (PEDVAXHIB / ACTHIB – primary series & not
administered after age 4 yrs
(7) IPV (thru age 18 yrs. only) primary series of 3 with
booster dose @ age 4 yrs
(8) Pneumococcal Conjugate (PCV13 / Prevnar 13) primary
series of 4 doses – not administered after age 5 yrs. unless child is in high
risk group
(9) Meningococcal Conjugate (MCV4 = Menactra- 2 yrs-55yrs
with risk factors per ACIP recommendations - / Menveo – 11-55 yrs) usually
administered @ age 11yrs. Those with risk factors need a script from their
primary care physician
(10) Rotavirus (ROTATEQ=RV5 / ROTARIX=RV1) must be started
before age 15 wks. & completed by age 8 mo.
(11) HPV (Gardasil=HPV4/ Cervarix=HPV2) Gardasil is licensed
for males & females with series started @ age 11 – can be given as early as
age 9 yrs. & must be completed before age 27 yrs.
(12) Hepatitis A – 2 doses – started @ age 1 yr. with minimum
of 6 mo. between doses
(13) MMR – age 1 yr. with booster @ age 4 yrs. – minimal
interval is 1 mo.
(14) MMRV (Proquad=MMR & Varicella) – age 1 yr. thru age
12 yrs. administered same as MMR with exception of minimal interval being 3 mo.
between doses due to Varicella component
(15) Varicella – age 1 yr. with booster @ age 4 yrs. –
minimal interval is 3 mo. unless client is age 13 or > at which time minimal
interval is 1mo.
(16) Pediarix (DTaP, HBV, and IPV) – 3 dose series - 6 wks. thru 7yrs of age – prior to the 7th birthday
(17) Pentacel (DTaP, HIB, & IPV) - 4 dose series – age
6 wks. thru 4 yrs – can be used as 1yr. old booster
(18) Kinrix (DTaP & IPV – 5th DTaP &
4th IPV - 4 yr. old booster dose only)
(19) Influenza – Seasonal vaccine. We do not stock DT
& do not use COMVAX OR TWINRIX
