THE FOLLOWING ITEMS ARE REQUIRED AT THE TIME OF VACCINE PICK UP:

  1. Valid prescription
  2. Photo ID
  3. Insurance card, Medicare card and/or Medicaid card, Personal Check, or sign payment agreement.
  4. Must bring an ice chest with cold packs to transport vaccine back to physician's office for administration.  Ice chest must be big enough to hold the ice & at-least a 6 pack of drinks
