*ALL Law Enforcement positions require a Personal History statement and a Law Enforcement background consent to be completed, notarized and submitted to the department upon completion of the application. Personal History statement and Law Enforcement background consent may be obtained below, on the Sheriff's website (www.polkcountyso.net) or in person at the Human Resources Office. Both of these documents must be notarized, which can be completed at Human Resources if needed.
**Non-Law Enforcement - All applications submitted for employment with Polk County must complete the background consent form listed below and submitted with the application, or it is not considered complete.
Notice: Any position which requires the ability or skill to type must also have a current typing test. You may take a typing test at your local Work Force Center. All applications and typing scores will be considered valid for 180 days.
